Dental Examinations

Dental examinations are due yearly. Regular dental cleanings and checkups can help detect dental health problems early.

Non-Remote: Active duty service members assigned to a military dental clinic (non-remote) should check with their dental clinic for information on their next appointment.

Remote: Active duty service members whose home and duty station are greater than a 50 mile radius from a military dental clinic (remote), receive benefits through the Active Duty Dental Program (ADDP) which include two routine exams and cleanings in a consecutive 12-month period. Officers utilizing ADDP must request an Account Control Number (ACN) prior to their appointment. You can generate an ACN here or by calling the United Concordia appointment scheduling line: 1-866-984-2337.

NOAA Corps officers should print and take the DD Form 2813 (Dental Readiness Form) to each dental appointment. The completed DD Form 2813 should be uploaded by the officer to their OPF. Medical Affairs will review the uploaded dental documents and update the officer's medical readiness.

NOTE: NOAA Corps officers are responsible for timely completion of dental examinations and submission of documentation to Medical Affairs.
